Kidney Transplant Experience Map
Partnering with the UCSF Kidney Transplant Team at the Connie Frank Transplant Center, SOM Tech worked closely with clinical and nursing staff, facilitating workshops to identify and prioritize opportunities to expand the use of technology to improve the kidney transplant patient experience.
SOM Tech's discovery findings have encouraged the Transplant Center to improve existing web and email touchpoints with enhanced analytics, develop a video/marketing program, create a tablet based patient education piece and implement follow-up telemedicine visits. This patient experience map is one artifact from our discovery work.
